This is not, as you might expect, the fourth in a series of energy drinks. It's actually a plastic bag filled with an energy drink, with a tube connected to it and ending in a needle.
I know what you're saying. You're saying "Wouldn't it kill you if you injected something carbonated into your veins?" And I'm replying "No. Not at all. In fact, it would be delightfully refreshing."

References
- The line about having something carbonated injected into your bloodstream killing you refers to air embolism, a condition where gas in the bloodstream can cause severe damage, including stroke or heart attack. The response is a play on the phrase "Would it kill you to..." which is generally used to ask someone to do something they don't necessarily want to do. Like "Would it kill you to clean your room?"
