An Empty Altar is located in Your Tomb, which is accessible when you are Actually Ed the Undying. Its sole purpose is to bear the Holy MacGuffin; recovering the MacGuffin and putting it back on the altar ends the run.

Clicking the Altar without the MacGuffin shows a message:

This little altar is where you keep the Holy MacGuffin. Or did, until that jerk <playername> stole it.
