You're fighting a pygmy witch surgeonYour typical witch doctors are generally witch GP's, or witch pharmacists. When a pygmy has a really serious ailment -- for example, if there's a demon trapped in their skull that needs to be let out -- they get referred to a witch surgeon to perform the operation.
This one seems pretty sure that you have a demon trapped in your skull. Or at least he thinks it's a possibility, and is willing to risk your life to find out.

Notes
- The surgeon drops only one item each time, with the selection based on what you don't already have (ref).
- Since all drops are conditional, a yellow ray is not effective.
- You must have Torso Awaregness (or Best Dressed) for the surgical apron to drop.
- You can get multiple instances of each drop by closeting the one you want more of.
