The majority of these hideously awesome-looking defacements of skin are drawn on you by the Pretentious Artist (after you complete his quest). To obtain a tattoo, you must be wearing the corresponding Outfit to that tattoo. For example, if you want the Hot and Cold Running Ninja Suit Tattoo, you have to visit the Pretentious Artist whilst wearing the Hot and Cold Running Ninja Suit.
Once you have obtained a tattoo, it is yours for life. You may swap which tattoo is on your body through your account menu. You may only have one tattoo on display and it is shown on your profile for all to see.
In addition to the outfit tattoos, you may also obtain tattoos through ascending multiple times. You can also receive tattoos for opting to take additional challenges when ascending, like going Hardcore and taking a dietary path such as oxygenarian. There are two extra tattoos you can get by eating delicious salad or drinking martinis. In addition, some special players have been given tattoos for getting real life KoL tattoos.
